How to register

If you wish to register as a new patient, please come to the surgery to collect a new patient registration form. Our friendly admin team will discuss what information we require in order to register.

When registering, you will be asked to complete a new patient questionnaire. It can often take a few weeks for your notes from a previous surgery to transfer to ourselves. The questionnaire allows us to meet any immediate requirements that you may have in regards to health conditions or medication. The forms is available above and can be filled in and emailed to enquiries.w91034@wales.nhs.uk. Alternatively, you may collect a copy and complete this with your registration form.

All of our new patients are offered a health check at the surgery with our Health Care Support Worker. During this health check, they will take the following information - 

  • Brief history of any medical conditions
  • Family history of medical conditions
  • Blood pressure
  • Height
  • Weight
  • Lifestyle

If you take any medication that is on a repeat prescription, we recommend you have one months supply from your previous surgery prior to leaving. When registering, please bring a copy of your prescription to enable our GP's to add this to your records. Failure to do so may result in a delay in your prescription.

All new patient registrations from 12th July 2018 will automatically OPT IN to receive text messages from the surgery, they will be used to remind you about appointments and reviews etc. Please let the practice know if you want to opt out.
